Veterinarian-Client-Patient Relationship (VCPR)
A relationship with the client and patient must be established via physical exam or farm visit before providing medical advice, prescriptions, or treatments.
Payment at Time of Service:
Horseshoe Mountain Veterinary Services requires payment when services are rendered, including consultations, surgeries, and medication dispensing.

Estimates: estimates are provided but are not final guarantees of the total cost.

Late Fees & Debt Recovery: accounts in arrears, such as interest charges (e.g., 1.5% monthly) or recovery of legal/collection costs.

Small vs. Large Animal: may require a deposit for farm calls or elective equine surgeries. Horseshoe Mountain Veterinary Services requires immediate payment for small animal services.
Appointment Cancellations & No-Shows
24 hours notice for cancellations of appointments and cancellation.
Fees may be charged for "no-shows" or late cancellations, particularly for farm visits where travel time is scheduled.
Horseshoe Mountain Veterinary Services is a Mixed Practice
There is a difference between in-clinic appointments (small animals) and ambulatory/farm calls (large animals). Note there are differences in pricing and services rendered between the two kinds of appointments and health practices.
Information Collection
Ownership & Medical Records
We (Horseshoe Mountain Veterinary Services) retains ownership of all medical records, including radiographs and lab results, though clients may request copies.
Medical information is kept confidential unless disclosure is required by law (e.g., animal abuse or zoonotic disease reporting).
We are in compliance with Utah State Laws and Veterinary Codes and Ethics.
Professional Conduct & "Zero Tolerance"
We (Horseshoe Mountain Veterinary Services) have a "Zero Tolerance Policy" for abusive behavior toward staff. We (Horseshoe Mountain Veterinary Services) practice reserves the right to terminate the relationship if this policy is violated.
